危重症患者血清胰岛素、C肽及血浆皮质醇与血镁关系的临床研究

Relationship between concentration of serum magnesium and concentrations of serum insulin Cpeptide and plasma cortisols in critically ill patients:Clinical investigation

对ＩＣＵ８０例患者血清镁与血清胰岛素、Ｃ肽和血浆皮质醇之间关系进行研究。发现血镁与胰岛素呈正相关（ｒ＝０．４０７，Ｐ＜０．０１）；而与血清Ｃ肽和血浆皮质醇无关。低血镁组血清胰岛素浓度低于正常血镁组（Ｐ＜０．０１）；两组间血Ｃ肽及皮质醇浓度无显著差异。高血糖组患者血胰岛素浓度较正常血糖组低（Ｐ＜０．０５）。作者认为：危重症患者血镁浓度受血清胰岛素影响较大，胰岛素与Ｃ肽均为胰岛素原的分解产物，而本结果血镁仅与血清胰岛素有关，故危重症患者体内可能存在胰岛素抗体或胰岛素拮抗物，干扰血清胰岛素测定，也影响了胰岛素的正常生物活性。

We studied the relationship between concentration of serum magnesium and concentrations of serum insulin,Cpeptide and plasma cortisols in 80 patients in the ICU,and discovered that serum concentration of magnesium was positively correlated with insulin (r=0 407, P <0 01),but not correlated with Cpeptide and plasma cortisols.The Patients with hypomagnesemia had a low concentration of serum insulin( P <0 01),but did not show significant differences in the concentrations of serum Cpeptide and plasma cortisols.The patients with hyperglycemia had a low concentration of serum insulin ( P <0 05) We considered that the concentration of serum magnesium was greatly influenced by serum insulin Insulin and Cpeptide were all products of decomposed insulogen Our results showed that serum magnesium was only correlated with serum insulin,so we speculated that there might be insulin antibody or antagonists in critically ill patients,which interfered with the determination of serum insulin and its normal biological activity The antibogy or antagonist of insulin didn't interfer with the determination of Cpeptide
