丙泊酚复合麻醉对腹腔镜胆囊切除术患者应激反应和免疫功能的影响

The Effect of Propofol Anesthesia on Stress Response and Immune Function of Laproscopic Cholecystectomy

目的 观察丙泊酚麻醉对应激反应和免疫功能的影响．方法 选择ASA1～2级，在丙泊酚静脉复合麻醉下经腹腔镜胆囊切除术20例．经颈外静脉分别于麻醉前、手术开始前、胆囊切下时、术毕拔管后抽血检测皮质醇、胰岛素、血糖、补体．结果麻醉后血糖（6LU）逐渐升高（P〈0．01），补体C3、CHS0逐渐降低（P〈0．01），皮质醇（COR）一过性降低后逐渐升高（P〈0．01），补体CA、胰岛素（INS）无明显变化．结论 丙泊酚复合麻醉不能完全抑制病人的应激反应，对补体参与的机体免疫功能有一定抑制。

Objective To investigate the effect of propofol anesthesia on stress response and immune function of laproscopic cholecystoctomy. Methods Corticosterone, glucose, complement concentrations in plasma of 20 ASA Ⅰ - Ⅱ patients under propofol combined anesthesia undergoing elective laproscopic cholecystectomy,were respectively determined before anesthesia, operation, gallbladder ablation and after de - intubation. Resuits After anesthesia, blood glucose increased while C3 and CH50 decreased gradually （ P 〈0. 01 ）. Corticosterone concentration was biphasic. CA and insulin had no significance. Conclusions The present result suggested that propofol anesthesia inhibited immune function participated in complement without inhibiting the stress response completely.
