BiPAP呼吸机在急性左心衰治疗中的应用

The treatment effects of BiPAP on acute left cardiac dysfunction in elderly

目的观察急性左心衰患者应用BiPAP呼吸机后临床变化及血流动力学改变,判定其疗效。方法对20例急性左心衰竭患者在常规药物治疗基础上应用BiPAP呼吸机经鼻罩双向正压通气,以流量触发,采用面罩式,呼吸模式(S/t),吸气相压力(IPAP)12～20cmH2O,呼气相压力(EPAP)3～8cmH2O,呼吸频率16～20次/分,吸氧流量为5～8L/min,根据病情确定通气时间及次数,同时与20例常规药物治疗组进行对比。结果20例急性左心衰患者应用无创通气后半至一小时,心率、血压、血氧饱和度、平均动脉压、血氧分压、呼吸频率均有非常明显的改善,治疗总有效率为96%。而对照组在相应时间治疗总有效率仅为73.3%。结论BiPAP呼吸机能快速纠正低氧血症,是急性左心衰竭的有效治疗方法。

Objective To study the clinical and hemodynamie changes in acute left cardiac dysfunction （ALCD） in elderly patients before and after the bilevel positive airway pressure（BiPAP） ventilation, Methods 20 ALCD patients were treated by BiPAP and regular medicine treatment. Breath mode（s/t）, inspiratory pressure（IPAP）12-20cmH2O. expiratory pressure（EPAP）3-8cmH2O, breath rate 16-20/min. oxygen flowS-SL/min. The duration and times of ventilation were adjusted by severity of symptoms of ALCD. The control group were treated by regular medicines. Results After the BiPAP ventilation. Heart rate. Blood pressure. SpO2. mean arterial blood pressure. PaO2. respiration rate were significantly improved. The total effective rate is 95% in the treatment group and 70% in the control group, Conclusions BiPAP ventilation can redress hypoxia quickly. It is a effective treatment on ALCD
