激素替代疗法对围绝经期综合征妇女症状及血脂的影响及意义

Effects on Climacteric Syndrome Treated by Hormone Replacement Therapy

目的探讨激素替代疗法(HRT)对围绝经期综合征妇女的临床症状及血脂的影响及意义。方法将67例患围绝经期综合征妇女分为两组,正常血脂组36例,高血脂组31例,应用倍美力0.625mg/d和安宫黄体酮4mg/d,观察应用HRT前后患者临床症状、性激素、血脂各项指标的变化。结果治疗前高血脂组雌二醇(E2)水平低于正常血脂组(P<0.01);应用HRT后,两组均出现围绝经期综合征临床症状较治疗前明显改善(P<0.01);两组均出现E2上升,促卵泡生成激素(FSH)和黄体生成激素(LH)下降(P<0.01);甘油三脂(TG)变化不显著(P>0.05),总胆固醇(TC)、低密度脂蛋白—胆固醇(LDL—C)下降,高密度脂蛋白—胆固醇(HDL—C)升高(P<0.05或P<0.01);高血脂组TC、LDL—C下降及HDL—C上升变化尤为明显。结论HRT能有效治疗围绝经期综合征,能预防及改善绝经后妇女的血脂紊乱,发挥保护心血管作用。

Objective Effects on climacteric syndrome and serum lipids level researched. Methods 57 eases were divided into normal serum lipids group[NG, 36 cases, total cholesterol（TC）〈Smmol/L] and high serum lipids greup（HG, 31 cases, TC≥6mmol/L）.All treated by conjugated cstrogen（0.625mg/d） and Mebroxyprogcsterne acetate （4mg/d）. Changes of clinical syndrome, sex hormone and serum lipids level were recorded. Results Estradiol（E2） of HG was lower than NG before curing. Significant difference was appeared in clinical syndrome after hormone replacement therapy（HRT）. In NG, difference was appeared in E2（inereased, P〈0.01）, high density lipoprotein cholesterol（HDL-C）（increased, P〈0.05）,foUicle stimulating hormone（FSH） and luteinizing hormone（LH） （decreased, P〈0.01）, TC and low density lipoprotein cholesterol（LDL-C）（decrcased, P〈0.05）. In HG, difference was appeared in E2 and HDL-C（increased, P〈0.01）, FSH, LH, TC and LDL-C（dacreased, P〈0.01）. Conclusions HRT can adjust serum lipids level and prevention of cardiovascular system. It does good effects on climacteric syndrome.
