腹腔镜胆囊切除术胆管损伤的预防措施(附250例临床分析)

Precaution of Bile Duct Injury during Cholecystectomy with Laparoscope

目的总结腹腔镜胆囊切除术中预防肝外胆管损伤的经验。方法回顾分析2002—11～2005—11间收殆250例行腹腔镜胆囊切除术的临床资料。结果手术时间20．120min，平均58min。住院时间3—15d。均痊愈出院，无1例肝外胆管损伤。结论掌握手术适应证，良好的手术操作技术，辨认正确解剖胆囊三角，是防止肝外胆管损伤的关键。

Objective To summarize the experience of precaution of bile duct injury during cholecystectomy with laparoscope.Methods The clinical materials of 250 cases who underwent cholecystectomy with laparoscope from November 2002 to November 2005 retrospectively.Results The operative time was from 20 min to 120 min （58 min in average） . All patients discharged with cured after hospitalized 3 - 15 d without bile duct injury. Conclusion The key point of precaution might be handle the indication, good operative skill, recognizing anatomy bile triangle.
