摘要
目的探讨试纸法尿胰蛋白酶原-2快速测定在急腹症中筛选急性胰腺炎的意义。方法对135例就诊的急腹症病人同时检测尿胰蛋白酶原-2及血、尿淀粉酶,并比较其特异性和敏感性。结果29例急性胰腺炎中有27例尿胰蛋白酶原-2阳性,敏感性为93.1%,106例非胰腺炎的急腹症病人中有9例尿胰蛋白酶原-2阳性,特异性为91.5%,其中3例重症急性胰腺炎中尿胰蛋白酶原-2全部阳性。而血淀粉酶敏感性为86.2%,特异性84.9%;尿淀粉酶敏感性82.8%,特异性82.1%。结论尿胰蛋白酶原-2是急腹症病人筛选急性胰腺炎快速而简便的方法,具有较高的特异性和敏感性。
Objective To explore the significance of urinary trypsinogen-2 test for screening acute pancreatitis in patients with acute abdomen. Methods Urinary trypsinogen-2 as well as serum and urinary levels of amylase were assayed in 135 patients with acute abdomen. Results The diagnosis of acute pancreatitis was established in 29 patients. Among them, 27 cases with posi- tive urinary trypsinogen-2 were found and the sensitivity was 93.1%. Nine case with false positive were found in 106 cases of non-pancreatitis patients and the specificity was 91.5 %. All of the three patients with severe acute pancreatitis showed a positive of the test. The sensitivity and specificity of serum amylase test were 86.2% and 84.9% respectively, and of urinary amylase test were 82.8% and 82. 1% respectively. Conclusion Urinary trypsinogen-2 test is an easy and rapid diagnostic method with high sensitivity and specificity.
