摘要
目的探讨定量组织速度成像结合常规超声心动图评价伊贝沙坦对自发性高血压大鼠左室重构和左室功能的影响。方法20只12周龄雄性自发性高血压大鼠(SHR)随机分为SHR组(n=10)和伊贝沙坦组(IRB组,n=10),IRB组每只大鼠予以伊贝沙坦50 mg.kg-1.d-1灌胃12周,以10只12周龄雄性Wistar大鼠作为空白对照,12周后分别进行超声心动图扫查。结果与对照组相比,SHR组的左室舒张末期内径、舒张末期容积明显下降(P<0.05),E/A比值明显增大,E峰减速时间明显延长(P<0.05),而左室相对重量则显著增加(P<0.01);左室长轴方向上二尖瓣环、侧壁各节段收缩及舒张期峰值速度明显下降(P<0.05),而射血分数变化不明显(P>0.05)。伊贝沙坦组M型及二维超声指标均有明显改善,二尖瓣环处速度指标亦有显著改善。结论伊贝沙坦能明显抑制高血压左室重构、改善左室功能,定量组织速度成像(QTVI)结合二维超声能较全面、精确、定量评价高血压左室重构及左室功能。
Objective To assess the effects of irbesartan on left ventricular remodeling and function in spontaneously hypertensive rats by quantitative tissue velocity imaging. Methods 20 twelve- week - old male spontaneously hypertensive rots were randomly assigned to two groups; SHR group( n = 10) and IRB group( n = 10), 10 twelve - week- old male Wistar Kyoto rats were as control group. Rats in IRB group was given a daily dose of 50 mg/kg irbesartan for 12weeks by garage, and echocardiographic scan was performed in each group 12 weeks later. Results Commpared with WKY group , the left ventricular end - diastolic diameter and end - diastolic volume in SRH group were both significantly decreased( P 〈 0.05). The left ventricular relative mass were significantly increased( P 〈 0.01). the parameters of diastolic in SHR group including The E/A ratio E peak descending velocity were all significantly increased( P 〈 0.05).The peak longitudinal velocity of systole and diastole in SHR group at mitral annulus and segments of left ventricular lateral wall were significantly decreased. ( P 〈 0.05). However, the ejection fraction in SHR group were not decreased obviously( P 〉 0.05). Commparod with SHR group, the parameters all about in irbesartan group were significantly improved. Conclusion tan has beneficial effects on preventing left ventrieular remodeling and improving left ventrieular function . Using quantitative tissue velocity imaging combined with two- dimensional echocardiography, the effects can be assessed comprehensively, accurately and quantitively.
