慢性前列腺炎患者EPS中细胞因子测定的临床意义

Clinical Significance of Determination of Levels of Several Cytokines(IL-1β, IL-2, IL-10 and TNF) in Expressed Prostatic Fluid (EPS) From Patients with Various Types of Chronic Prostatitis

目的:探讨前列腺液(EPS)中白细胞介素-1β(IL-1β)、白细胞介素-2(IL-2)、白细胞介素-10(IL-10)及肿瘤坏死因子-α(TNF-α)水平与不同类型慢性前列腺炎患者之间的关系。方法:应用放射免疫分析(RIA)对86例慢性前列腺炎患者EPS中IL-1β、IL-2、IL-10、TNF-α含量进行了检测。结果:慢性前列腺炎组IL-1β、IL-2、TNF-α浓度均明显高于对照组(P均<0.05或0.01),IL-10浓度明显低于对照组(P<0.01)。慢性细菌性前列腺炎(CBP)组和慢性非细菌性前列腺炎/慢性骨盆疼痛综合征(CPPSⅢA)组EPS中IL-1β、TNF-α浓度明显高于慢性非细菌性前列腺炎/慢性骨盆疼痛综合征(CPPSⅢB)组(P<0.05或0.01),IL-10浓度明显低于CPPSⅢB(P<0.01)。结论:EPS中IL-1β、IL-2、IL-10、TNF-α在慢性前列腺炎的发病过程中起重要作用,检测EPS中的IL-1β、IL-2、IL-10、TNF-α水平是诊断慢性前列腺炎有价值的指标。亦是CPPSⅢ诊断与分型的重要指标。

Objective To investigate the levels of several cytokines in EPS from patients with different types of chronic prostatitis. Methods The EPS levels of IL - 1β, IL - 2, IL - 10 and TNF were determined with RIA in 86 patients with various types of chronic prostatitis as well as in 30 controls. Results The EPS levels of IL - 1β, IL - 2 and TNF in all the 86 patients with various types of chronic prostatitis were significantly higher than those in the controls （P 〈0.05 or P 〈0.01 ） while the IL- 10 levels were significantly lower （ P 〈 0.01 ）. The EPS levels of IL - 1β and TNF in patients with chronic bacterial prostatitis （ n = 26 ） and chronic non - bacterial prostatitis/chronic pelvic pain syndrome A （ CPPS ⅢA, EPS WBC 〉 10/HP, lecithin body 〈 ＋ ＋ ） （ n = 31 ） were significantly higher than those in patients with CPPS Ⅲ B （ WRC 〈 10/HP, lecithin body normal, n = 29） （ P 〈 0.05 or P 〈 0.01 ）, while the IL - 10 levels were significantly lower （P 〈 0.01 ）. Conclusion EPS levels of the cytokines IL -β, IL - 2, TNF and IL - 10 might be of value for diagnosis and classification of chronic prostatitis.
