血清肌钙蛋白Ⅰ对急性一氧化碳中毒早期心肌损伤的诊断意义 被引量:1

The Diagnostic Significance of Plasma Cardiotroponin I(cTnI)in Early Cardiac Damage with Acute Carbon Monoxide Poisoning

摘要 目的探讨心肌肌钙蛋白 I(cTnI)在急性一氧化碳中毒(ACOP)患者心肌损伤早期诊断中的意义及其对心脏事件发生率的检测作用、方法将48例本院 ACOP 患者分 A 组(轻度)、B 组(中度)、C 组(重度)三组,于接诊后3h 内检测 cTnI、心肌酶学、心电图(ECG),比较 cTnI 和其他单项指标或综合指标对三组 ACOP 患者心肌损伤早期检出率的差异性。结果 C 组 cTnI 明显高于 A 组、B 组,差异有显著性(P<0.01),B 组亦高于 A 组,差异性有显著性(P<0.05),心肌酶学及 ECG 于各组问无差异性(P>0.05)。cTnI 升高的例数显著高于单一或综合指标异常例数(P<0.05)。结论应用血清 cTnI 检测方法能提高 ACOP 患者心肌损伤的早期检出率,其灵敏度高于心肌酶学和心电图检查。 Objective To explore the diagnostic significance of plasma cardiotroponin I (cTnl) in early cardiac damage with acute carbon monoxide poisoning and it' s detection effect of heart events incidence. Methods 48 cases with ACOP were divided into group A (mild), group B( moderate), group C(severe). cTnl, cardiac enzyme and electrocardiogram(ECG) performed within 3 hours after admission. Plasma cTnI was compared as single index and the synthetic index, in order to obtain the difference with early detectable one for cardiac damage in patients with ACOP. The sensitivity and specificity of predicting the heart events also would be found. Results The cTnI level in group C was significance higher than that in group B ( P 〈 0.01 ), the same was existed in group C and group A ( P 〈 0.05 ). There was no significance difference to cardiac enzyme and ECG among the all groups ( P 〉 0.05 ). The eases with elevated cTnI were significantly higher than those cases with abnormal single or synthetic index( P 〈 0, 05 ). Conclusion Mesufing plasma CTnI can improve the detectable rate to cardiac damage in patients with ACOP, and it's sensitivity were better than that of cardiac enzyme and ECG .
