复合骨髓间充质干细胞的组织工程皮肤治疗慢性难愈性溃疡探讨 被引量:12

摘要 目的探讨复合骨髓间充质干细胞(MSCs)的组织工程(TE)皮肤在修复皮肤缺损中的疗效,为临床治疗类似疾病提供新方法。方法利用组织工程学方法,以牛胶原为支架,自体MSCs为种子细胞,构建TE皮肤。对1例双下肢溃疡15年的患者进行移植治疗,观察疗效。结果移植后1周溃疡边缘变平,健康的肉芽组织形成,2周后皮岛扩大,边缘形成了较薄的上皮组织。4周后溃疡面积缩小40%,9周后溃疡面积缩小90%。结论构建的复合骨髓间充质干细胞的组织工程真皮移植后,提高了愈合速度,获得了较理想的创面愈合效果。为临床治疗类似溃疡型疾病提供了新的治疗思路。 Objective To investigate the practicability of repairing ulceration of chronic nonhealing ulceration with tissue engineering dermis combinded with bone mesenchymal stem cells, so as to provide a new method for clinical analogical skin defect repair in the future. Methods Using the method of tissue engineering, calf collagen matrix as supporter, bone mesenchymal stem cells of patient as original cells,tissue engineering dermis was constructed. We transplanted the tissue engineering dermis to a 26-year-old woman with a 15-year history of lower extremity ulcers of chronic nonhealing ulceration, and observed the therapeutic efficacy. Results One week after transplantation,the edge of ulcer flattens and fresh granulation tissue was shaped,two weeks later the derm was expanded and thinner epithelia tissue was formed on the margin. 4 and 9 weeks after transplantation, the areas of ulcer decreased 40% and 90% repectively. Conclusion Transplanting the the tissue engineering dermis combinded with bone mesenchymal stem cells to a patient with chronic nonhealing ulceration can accelerate cutaneous wound healing and enhance the wound healing quality,which may open a new way to repair clinical same skin defect.
