摘要 目的研究非小细胞肺癌(NSCLC)患者行 PET 和 CT 检查的成本效益比,寻找更低成本效益比的检查。方法采用决策分析法,对国内现有 NSCLC 患者 PET 及 CT 诊断效率研究中的数据进行分析,比较其正确检出费用、正确分期费用和正确治疗费用。结果①就检出率而言,CT 的成本效益比更低;②就分期准确性而言,CT 的成本效益比仍较 PET 低;③就诊疗费用的成本效益比而言,PET 较 CT 为低;④PET 与 CT 相比,其优势在于诊断Ⅰ期的患者。结论 CT 适合 NSCLC 的筛选,而 PET 适用于临床分期,指导临床治疗。 Objective To evaluate the cost-effectiveness of PET and CT application for diagnosis of non-small cell lung cancer (NSCLC) in China. Methods Using decision analysis method the diagnostic efficiency of PET and CT for diagnosis of NSCLC in china was analysed. And also the value of cost for accurate diagnosis ( CAD), cost for accurate staging (CAS) and cost for effective therapy (CAT) was calculated. Results ①For the accurate diagnosis, CT was much more cost-effective than PET. ②For the accurate staging, CT was still more cost-effective than PET.③For the all over diagnostic and therapeutic cost, PET was more cost-effective than CT. ④The priority of PET to CT was for the diagnosis of stage I NSCLC. Conclusion For the management of NSCLC patient in China, CT is more cost-effective for screening, whereas PET for clinical staging and monitoring therapeutic effect.
