摘要
目的建立一种更简单有效的大鼠左肺原位异体移植模型。方法持续供肺肺动脉低压灌注4℃的低钾右旋糖苷(LPD)液,采用连续外翻法褥式缝合吻合支气管膜部、肺静脉、肺动脉,快速建立大鼠左肺原位移植模型。结果60只SD大鼠接受左肺移植,平均吻合时间(36.2±9.6)min,手术成功率91.7%。术后抗免疫治疗,1周存活率为70.9%;2周存活率为50.9%;4周存活率为21.8%。术后肺动脉栓塞1例,死于术后第3天,余所有受体大鼠没有发现吻合口狭窄及栓塞。结论该新的模型对于研究肺移植供肺保护、缺血再灌注损伤以及急、慢性排斥反应等近远期的相关问题,具有简单可靠、存活时间长的优点。
[Objective] To establish a new simple, safe, and available experimental model of rat lung orthotopic allograft transplantation by using a new technique and study its prospect of application. [Methods] Sixty left lung transplantation were performed by using 120 Sprague Dawely rats. In this orthotopic left allograft lung transplantation model, the donor lung was perfused continuously with lung preservation solution (low potassium dextran, LPD) at 4℃ through the pulmonary artery when anastomosis. The pulmonary artery, pulmonary vein and membrane section of bronchus were successively mattress anastomosed in lumen. [Results] Transplants were completed in 55 rats (91.7%), the mean time for anastomoses was 36.2±9.6 min, All survival rats were given immunosuppressive therapy. The survival rates of rats who received transplants were 70.9% at 1 week, 50.9% at 2 week and 21.8% at 4 week. Of the rats who received transplants, 1 died at 3 day after operation and found pulmonary thrombosis, and others had no sign of stenosis and thrombosis. [Conclusion] Rat orthotopic left lung transplantation with successive anastomoses in cavity is a valid, reproducible, single and cheap model for studying donor lung preservation, isehemia-reperfusion, acute rejection and chronic rejection about lung transplantation
