新鲜羊膜与自体角膜缘干细胞移植治疗翼状胬肉疗效分析

Curative Effect Analysis on Fresh Amnion with Autogenous Corneal Limbus Stem Cell Transplantation in Treatment of Pterygium

目的：观察新鲜羊膜移植与自体角膜缘干细胞移植治疗翼状胬肉的临床疗效。方法：随机选择翼状胬肉患者64例（64只眼）分为两组，各32例，一组为胬肉切除联合新鲜羊膜移植；二组为胬肉切除联合自体角膜缘干细胞移植，观察术后角膜愈合，角膜缘新生血管夏球结膜是否有结膜翼状胬肉样组织增生。结果：随访3个月-25个月，一组2例复发（6．3％），二组无复发病例。结论：羊膜移植目的在于恢复角膜缘基质微环境，而角膜缘干细胞移植在于恢复角膜缘干细胞的数量，因此，两种手术方法均可行。但角膜缘干细胞移植手术方式系自体眼表组织移植，手术简便安全，成功率高，是处理翼状胬肉复发的最佳手术方式。

Objective： To survey the clinical effect in treatment of pterygium by fresh amnion with autogenous corneal limbus stem cell transplantation. Methods：A total of 64 patients （64 eyes） with pterygium were randomly selected and divided into random group A （ pterygium excision with fresh amnion transplantation） and group B （ ptery- gium excision with autogenous corneal limbus stem cell transplantation） so as to observe whether the postoperative corneal healing, neoformative blood vessel of corneal limbus and bulbar conjunctiva have conjunctival pterygium tissue hyperplasia, Results. A follow-up of 3-25 months was made. There were 2 recurrences among 32 cases in group A （6.3 % ） and no relapses in group B, Conclusions. The purpose of amnion transplantation is in that it can restore the matrix microenvironment of corneal limbus, while corneal limbus stem cell transplantation is in that it can restore the number of corneal limbus stem cell, ＇Both of methods, therefore, can be used. But it is the best operative pattern that corneal limbus stem cell transplantation treat recurrent pterygium because it is autogenous oculofacial issue transplantation. The operation is safe and easy. And the rate of success is high.
