摘要 目的研究介入性超声在乳腺疾病诊断和治疗中的应用。方法196例乳腺疾病患者均经超声引导自动活检和病理组织学诊断。其中,54例良性乳腺结节性病变患者又经超声引导经皮注射无水酒精硬化治疗。治疗后,对54例患者随访了0.5a~5a,并评价其治疗效果。结果196例中,超声引导自动活检获得病理组织学明确诊断者181例,诊断率为92.3%。54例良性乳腺结节病变患者中,在超声引导下经皮注射无水酒精硬化治疗后,结节消失或仅留痕迹者45例,治愈率为83.3%。研究发现,治疗效果与结节的大小及物质性质有明显的相关性(P<0.05)。本组病例中未发生与操作相关的严重并发症。结论介入性超声在乳腺疾病的诊断和治疗中具有重要的临床应用价值。 Objective To investigate the applications of interventional ultrasound in the diagnosis and treatment of breast diseases. Methods A hundred and ninety - six patients with breast diseases underwent ultrasound guided automatic biopsy and histopathologic diagnasis, of them, fifty - four patients with histopathologically proved benign breast nodules underwent additional sclerosed therapy which was performed by ultrasound guided percutaneous ethanol injection(PEl) . Following the sclerosed therapy, all patients were followed up for 6 months to 5 years in order to assess the therapeutic effect. Results Through ultrasound guided automatic biopsy, 181 of 196 patients with various breast diseases obtained definite histopathologic diagnosis so that the diagnostic accuracy was 92.3 %. After ultrasound guided PEI, the disappearance of nodular lesions or only remainders of their vestiges were found in 45 cases so that the cured rate was 83.3 %. The study also shown that, there was significant correlation between therapeutic effect and nodule' s size or physical property ( P 〈 0. 05). There were no severe complications related to the procedure in the group. Conclusion Interventional ultrasound has important usefulness in the diagnosis and treatment of breast diseases.
