急性非等容血液稀释对血小板及凝血功能的影响(附26例报告) 被引量:1

摘要 目的探讨急性非等容血液稀释(ANIH)对血小板各参数和凝血功能的影响。方法选择脊柱择期手术26例,于麻醉诱导前采集全身血容量15%~20%的血液,然后在30min内补充2倍采血量的6%羟乙基淀粉与乳酸林格氏液(其容积比为2:1)进行ANIH。分别于采血前、采血毕、稀释毕、术毕测定Ph、MPV、PDW、BPR、BT、ACT、PT、APTT和FIB。结果Plt在稀释毕和术毕明显降低(P〈0.01);PDW和MPV在稀释后轻度升高(P〉0.05);稀释毕Plt与PDW和MPV均呈负相关(r=-0.731,P〈0.01;r=-0.819,P〈0.01),PDW和MPV呈正相关(r=0.871,P〈0.01);PT和APTT在稀释毕和术毕均延长(P〈0.01),ACT术毕延长(P〈0.01),但均在正常范围;术毕FIB增加(P〈0.01),亦在正常范围。结论ANIH对血小板的结构和功能无明显影响,凝血功能正常。可作为一种新的血液保护手段用于临床。
