摘要
目的研究脑出血急性期血肿内的白细胞浸润及其与细胞间黏附分子-1(ICAM-1,CD54)表达的关系。方法33例行颅内血肿微创手术的脑出血患者,手术时留取血肿液及静脉血,测定其白细胞数,观察血肿内白细胞浸润情况,并用流式细胞术(FCM)检测其ICAM-1的表达,对两者进行相关分析。结果脑出血组血肿液、静脉血与健康对照组静脉血中粒细胞及淋巴细胞表面ICAM-1的表达及白细胞数间的差别均有显著性意义(P<0.01),组间比较差别亦均有显著性意义(P<0.01)。脑出血患者血肿内白细胞浸润与其血肿内粒细胞及淋巴细胞表面ICAM-1的表达呈正相关(P<0.05),与其静脉血中粒细胞及淋巴细胞表面ICAM-1的表达呈正相关(P<0.01)。结论脑出血急性期ICAM-1的表达上调,ICAM-1的增高表达与血肿内的白细胞浸润相关。ICAM-1参与了血肿内的炎症反应。
Objective To investigate the leukocyte infihration inside the hematoma and its correlation with the expression of intercellular adhesion molecule - 1 ( ICAM - 1 ) of patients with cerebral hemorrhage. Methods In 33 patients with cerebral hemorrhage treated with minimally invasive surgery, the expression of ICAM -1 and leukocyte count in intracerebral hematoma fluid and venous blood were detected with cytometry and flow cytometry (FCM) . The correlation between the two variables was also analyzed. Results The expression of ICAM - 1 was higher in patients with acute cerebral hemorrhage than that in health individuals, and the expression of ICAM - 1 in venous blood was higher than that in intracerebral hematoma fluid in patients with acute cerebral hemorrhage ( P 〈 0. 01 ). There was a positive correlation between the count of infiltrating leukocytes inside the hematoma and the level of ICAM - 1 expressed on the Granuloeytic cells and the Lymphoeytes surface ( P 〈 0.05 ), and there was a significant positive correlation between the count of infiltrating leukocytes inside the hematoma and the level of ICAM - 1 expressed on the granulocytle cells and the lymphocytes surface in the venous blood of cerebral hemorrhage patients ( P 〈 0. 01 ). Conclusion In the acute phase of cerebral hemorrhage, higher expression of ICAM - 1 appears, which is closely related with the leukocyte infiltration inside the hematoma. ICAM - 1 takes part in the inflammatory reaction.
