超声声学密度定量技术评价扩张型心肌病患者左室功能的研究

摘要 目的应用超声声学密度定量(AD)技术评价扩张型心肌病(DCM)患者的心肌纤维化程度,并探讨心肌纤维化程度与左室收缩功能的相关性。方法使用AD技术对60例DCM患者(DCM组)及55例查体健康者(对照组)采集心肌背向散射积分数据:室间隔中部及左室后壁校正的IBS(IB%)、CVIB。结果与对照组比较,DCM组室间隔中部、左室后壁校正的IBS(IB%)明显增大(P<0.05和0.01);IBS曲线形态异常;CVIB明显减低(P<0.01)。与对照组比较,DCM组IB%越大,其左室射血分数越低。结论AD技术为定量评价DCM患者的心肌纤维化程度和左室收缩功能提供了新的途径。 Objective To evaluate the degree of myocardioa fibrosis in patients with dilated cardiomyopathy (DCM) using acoustic densitometry (AD) and to evaluate the relationship between fibrosis and left ventricular (LV) systolic function. Methods Myocardial ultrasonic integrated backscatter (IBS) in 60 DCM patients and in 55 healthy subjects as the control group were measured by AD including corrected myocardial IBS (IB%)and CVIB in IVS and LVPW. Results Compared with the control group, time-averaged values of IB% in IVS and LVPW in patients with DCM were significantly elevated (P〈0. 05 and 0. 01 respectively) ; patterns of time-IBS curve turned out to be lower and CVIB values were decreased greatly (P〈0. 01); the more the IB% was elevated in DCM, the lower the LVEF was decreased. Conclusions AD technique may be used as a new method to define the relationship between the myocardial fibrosis and left ventricular systolic function quantitatively.
