摘要
目的为了评价实时三维超声心动图(RT-3DE)诊断先天性主动脉瓣下隔膜的价值。方法经手术或CT/MRI证实的28例先天性主动脉瓣下隔膜患者接受了二维、彩色多普勒和RT-3DE成像。结果二维超声显示先天性主动脉瓣下隔膜,67.9%(19/28)可清晰显示,17.8%(5/28)可疑,14.3%(4/28)显示不清晰;在RT-3DE上,85.7%(24/28)可清晰显示,14.3%(4/28)显示不清晰。显示清晰者不仅可在左室流出道显示片状回声附着,其大小、形态、分布范围及其与主动脉瓣、二尖瓣和室间隔左室面的关系也更直观。结论在先天性主动脉瓣下隔膜的诊断中,RT-3DE可比二维超声提供更多的形态学信息。
Objective To evaluate the effect of real-time three-dimensional echocardiography (RT-3DE) on diagnosis of congenital discrete subaortic membrane. Methods A total of 28 patients with congenital discrete subaortic membranes,who were accessed by surgical operation or CT/MRI, undertook two-dimensional echocardiography, color Doppler flow imaging and RT-3DE. Results 67.9%(19/28)of congenital discrete subaortic membranes was clear, 17.8%(5/28) suspicious, and 14.3%(4/28) unclear on two-dimensional echocardiography. While 85.7%(24/28) was vivid and 14.3%(4/28) cloudy on RT-3DE. Attached discrete subaortic membrane at left ventricular outflow tract not only was displayed clearly, but its size, shape, scope and spatial connection with aortic valves, mitral valves and left side of ventricular septum were more intuitive if the subaortic membrane was visual on RT-3DE. Conclusion RT-3DE may provide more abundant morphologic information about congenital discrete subaortic membrane compared with two-dimensional echocardiography.
