术前超选择性肾动脉化疗栓塞对小肾癌保肾手术的价值

The value of superselective chemotherapy and embolizaation of rennal in the nephron sparing surgery of small renal carcinma

目的探讨术前超选择性肾动脉化疗栓塞对小肾癌保肾手术的影响及价值。方法29例小肾癌保肾手术患者中13例行术前超选择性肾动脉化疗栓塞,另16例作为对照组分析效果。结果术前介入治疗组1/3肾脏手术切除率92%,3年复发率0,无术后出血及尿漏发生,对照组分别为37%和18.9%,差异显著,(P<0.05),术后出血和尿漏发生率为25%和31%,两组差异显著,(P<0.05)。结论术前超选择性肾动脉化疗栓塞可以提高小肾癌保肾手术切除率,降低并发症和复发率。

Objective：To study the influence and value of superselective chemotherapy and embolization of renal artery in the treatment of small renal carcinoma before nephron sparing surgery. Methods：Twenty nine cases of small renal carcinoma were randomized into two groups. The experimental group （13 cases） were treated by superselective chemotherapy and embolization of renal artery before nephron sparing surgery,and the control group （ 16 cases） were treated with nephron sparing surgery alone. Results：There were significant differences between these two groups in the cases of one third renal resect, bleed volume in or after operation, complication and 3 - year recur rate. Conclusion： Superselective chemotherapy and embolization of renal artery before nephron sparing surgery would incrdase the renal retain rate and reduce complications and 3 -year recur rate.
