摘要
目的探讨HCV感染者体内Ig特异性激活补体类循环免疫复合物(Ig/C3-CIC)的病理生理意义。方法采用捕捉法ELISA,检测80例HCV感染者血清中的IgG、IgM和IgA特异性激活补体类循环免疫复合物(即IgG/C3-CIC、IgM/C3-CIC和IgA/C3-CIC),并分析三类Ig/C3-CIC与患者的性别、年龄、病程及谷丙转氨酶(ALT)的关系。结果HCV感染者体内IgG/C3-CIC、IgM/C3-CIC和IgA/C3-CIC的阳性率分别为65.0%、61.3%和45.0%。其中,IgM/C3-CIC和IgG/C3-CIC在ALT升高组显著高于ALT正常组,IgA/C3-CIC与ALT的升高与否无关;IgG/C3-CIC和IgA/C3-CIC的阳性率随病程的迁延而显著升高,IgM/C3-CIC与病程无明显关联;三类Ig/C3-CIC与性别和年龄均无关。结论HCV感染者体内Ig/C3-CIC可能是直接参与肝脏损害及慢性化的因素,不同Ig类型的Ig/C3-CIC具有不同的病理意义。
Objective To elucidate the significance of circulating immune complexes with specific Ig and C3 (i, e, IgG/C3-CIC ,IgM/C3- CIC and IgA/C3-CIC) in HCV infected individuals. Method Using a capture ELISA method, three Ig/C3-CICs were assayed in the serum of HCV-infected individuals, Results The positive percentages of the three CICs were 65. 0%, 61. 3%, and 45. 0% respectively. By statistical analysis, it was found that IgG/C3-CIC and IgM/C3-CIC were markedly higher in the group with abnormal ALT than those in the group with normal ALT, whereas IgA/C3-CIC was independent of ALT. IgG/C3-CIC and IgA/C3-CIC were positively correlated with the progression of disease, but IgM/C3-CIC was not. Furthermore, both of the age and the gender were not correlated with the three Ig,/C3-CICs. Conclusion These findings showed that the Ig/C3-CICs might directly participate in the process of liver injury and conversion to chronic infection, and the heterogeneity of Ig/C3-CICs would lead to different pathological significance.
