重症支气管哮喘无创正压通气的疗效评价

Evaluation of Noninvasive Ventilation in Treatment of Patients with Severe Asthma

目的:无创通气在重症支气管哮喘患者的疗效评价。方法:观察20例应用无创通气治疗重症哮喘患者治疗前,治疗后4 h,24 h及72 h的血气指标及部分生理指标变化;治疗前后肺功能PEF,FEV1％的变化。与20例使用常规方法治疗的重症哮喘患者作了对比。结果:无创通气组的血气指标(pH,PaO2,PaCO2)。肺功能指标(呼吸频率,PEF,FEV1％)以及心率的改善均优于治疗前及对照组(P<0．05)。结论:应用无创正压通气治疗危重症哮喘效果好,并发症少,患者易于接受,值得临床推广。

Objective. To evaluate the effectiveness of noninvasive ventilation in patients with severe asthma. Methods：40 cases with severe asthma patients were assigned into two groups： the BiPAP group and the conventional group. Change of pH, PaCO2, PaO2, RR and HR were measured before the treatment and 4 hours, 24huors , 72hours after the treatment, and the comparison was made between the two groups. Results： Significant differences were found in the BiPAP group in the improvement of pH and PaO2, PEF and FEV1% and decreased value of PaCO2 ,HR, RR before and after the treatment , compared with the conventional group（P〈0. 05）. Conclusion： The noninvasive ventilation in treating severe asthma is effective, well-compliant and has few complication.
