110例纵折后牙粘结再植治疗的临床分析及疗效观察

The Clinical Analysis on 110 Cases of Vertical Fractured Posterior Tooth by Bond and Replant Therapy

目的:通过对纵折后牙粘结再植治疗病例的回顾性分析和跟踪随访,对纵折后牙的发病年龄、好发牙位和诱发因素等进行总结,并对其疗效做出评价。方法:收集近4年来完成的110例纵折牙粘结再植治疗患者的临床资料,对患者的性别、发病年龄、好发牙位和诱发因素等进行回顾性分析．同时对术后1年和3年以上的患者进行临床跟踪随访,对患牙进行牙体、牙周、松动度、咀嚼功能和X线摄片等方面检查,从而对其中长期疗效做出评价。结果:纵折后牙以40～59岁患者最多,以上颌第1磨牙最为多见,占30．91％,其中死髓牙为75．46％．粘结再植术3年的有效率达到了90％以上, 78.95％的患牙的咀嚼功能恢复良好,无松动、根尖及根分叉骨质正常。结论:折裂牙的粘结再植术保存了相当部分患牙, 对恢复咀嚼功能,维持牙列完整,保持患者的良好的生活质量等均具有一定的积极意义,是目前对后牙纵折一种较好的治疗方法。

Objective.. To summarize the favorable age, position and provoking factors of vertical fractured posterior teeth （ VFPT） and appraise the curative effects through the retrospective analysis and follow-up of these teeth treated by Bond and Replant therapy. Methods.We collected 110 cases conducted in the latest 4 years. Favorable age, tooth position and provoking factors were analyzed retrospectively. Meanwhile, we followed up patients for one and three years after treatment, examining the tooth and periodontal condition, the tooth loosing extent, chewing function , X-ray and etc. Based on the aforementioned data, we made medium and long term appraisal for the curative effect of Bond and Replant therapy. Results： Vertial fractured posterior teeth were observed mostly in 40 to 59 age group with upper first molar the most often involved （account for 30. 91%, in which 75. 46% are dead pulp teeth）. The effectiveness rate of 3 years after treatment was above 90%. And 78. 95% of the treated teeth exhibited good chewing function, firm standing, normal bone structure around root apex and furcation, etc. Conclusion：The Bond and Replant technique for fractured tooth can be used in some difficult cases to preserve the tooth and is quite meaningful to rebuild chewing function, maintain the integrity of the denture and promot the quality life of the patients. At present, it is a good choice for vertical fractured posterior tooth.
