摘要
目的探讨血小板膜P2Y_(12)受体介导的血小板活化及凝血激活与冠心病的关系及临床意义。方法采用病例对照研究,检测238例中国四川地区[冠心病(coronayatheroscleroticheartdisease,CHD)组177例,对照(control)组61例]血小板最大聚集度(Maximumplateletaggregationratio,MAR)、血浆P选择素(granulemembraneprotein,GMP140)及各项凝血指标。结果冠心病组与对照组比较,MAR、GMP140、纤维蛋白原(Fibrinogen,FIB)水平显著升高,凝血酶原时间(prothrombintime,PT)显著降低(P<0.01);多元Logistic回归分析逐步校正后,GMP140水平和冠心病密切相关(r=0.69,P<0.1)。结论血小板膜P2Y12受体活化介导的血小板高活状态引起的血浆高GMP140水平在冠心病的发生发展中起重要作用。
Objective: To study the association of platelet membrane receptor P2Y12 mediated platelet activation and sigmal transmit pathway with coronary atherosclerotic heart disease. Methods: A total of 177 patients with coronay atherosclerotic heart disease ( CHD) and 61 controls with no history of coronary heart disease, thrombogenic and hemorrhagenic disrases in Sichuan region were studied by casecontrol study, Measured: with ADP induced platelet aggregation, plasma granule membrane protein (GMP140) and regulation cruor items. Results: Compared with controls, Maximum platelet aggregation ratio (MAR), plasma GMP140 and Fibrinogen (FIB) levels in CHD patients were significantly higher (P 〈 0.01 ), prothrombin time (FF) was significandy lower ( P 〈 0.01 ). Correlation analysis revealed that plasma GMPI40 was positively associated with coronay atherosclerotic heart disease (r =0. 69, P 〈 0.1). Conclusion: High level of plasma GMP140, which was induced by platelet surface receptor P2Y12 activation mediated high activation of platelet played important role of occurrence and progress of coronay atherosclerotic heart disease.
