摘要
目的:探讨18氟-脱氧葡萄糖(18F-FDG)正电子发射计算机体层摄影(PET-CT)在卵巢癌术后监测中的应用价值。方法:16例卵巢癌患者经肿瘤细胞减灭术和术后化疗或放疗,临床完全缓解6个月以上。行18F-FDG PET-CT检查,结果与手术病理结果对照。结果:18F-FDG PET-CT发现了9例患者,有复发或转移,共发现病灶直径0.3~3.2cm 23处病灶。手术病理证实9例(56%)无假阳性,无假阴性,诊断准确率100%;诊断复发或转移灶的敏感性、特异性、准确率、阳性预测值和阴性预测值分别为78%、78%、78%、90%和58%。PET-CT显示与术中所见病灶的大小、部位均一致。结论:18F-FDGPET-CT在卵巢癌的术后监测中有很高的诊断价值。
Objective:To investigate the efficiency of ^18 F fluoro-2-deoxy-glueose(^18F-FDG) positron emission tornographie-CT for detecting recurrent ovarian carcinoma.Methods:Sixteen women with ovarian carcinoma treated with primary cytoreductive surgery and followed up with chemotherapy or radiotherapy underwent fluorodeoxyglucese(^18F-FDG) PET-CT and compared with surgical pathology after surgical second-look to determine the diagnostic accuracy of PET-CT in the evaluation of disease status.Results:Nine (56%) of 16 patients had persistent tumor at histologic analysis after surgical second-look, and seven (44%) had no histologically proved tumor. The total that was positive for tumor cells at histologic analysis was 23 lesions; maximum diameter of these lesions was 0.3 - 3.2cm. ^18 F-FDG PET-CT found all 9 patients with persistent tumor successfully. The overall lesion-based sensitivity, specificity, accuracy, positive predictive value and negative predictive value of ^18 F-FDG PET-CT were 78% ,78% ,78% ,90% and 90%. ^18 F-FDG PET-CT imaging findings consistent with surgical second-look in the size and site of tumor. Conclusion:^18F-FDG PET-CT has high value for monitoring and locating the recurrence of ovarian carcinoma.
