Ahmed青光眼阀植入联合手术治疗难治性青光眼临床分析

Combined AGV with other intraocular surgery for refractory glucoma

目的探讨Ahmed青光眼阀(AGV)植入联合手术治疗难治性青光眼的临床疗效及并发症。方法对24例(24只眼)难治性青光眼应用AGV及其联合手术治疗,其中10只眼为原发性青光眼行滤过性手术后眼压失控,8只眼为葡萄膜炎继发青光眼,6只眼为新生血管性青光眼。结果术后随访3～24月,平均18月。术后总有效率为87．5％,术前平均眼压(44．59±13．04)mmHg,术后平均眼压(16．52±4．61)mmHg,,差异有统计学意义 (P<0．01)。并发症为早期低眼压、浅前房、引流口堵塞、引流盘包裹、眼内出血等。结论 AGV植入及其联合手术是治疗难治性青光眼安全有效的途径。

Objective To describehgh the clinical results and neopathy of Ahmed glaucoma valve（AGV） implantation in refractory glaucoma. Methods to document 24 eyes from 24 patients with refractory glaucoma who undergone AGV implantation or combined AGV implantation with other intraocular surgery, of which 10 cases was high tension after strain surgery. 8 cases was uveitis secondary glaucoma. 6 cases was neovescular glaucoma. Results The follow-up ranged from 3 to 24 months ,mean 18 months. Success rate of operation was 87.5 % Respectively, Intraocular pressure was reduced from a preoperative mean of （44.59 ± 13.04mmHg） to a preoperative mean of（ 16.52 ±4.61mmHg） （ P 〈0.01 ）. The major complications included early hypotony, shallow anterior chamber, tube blockage, encapsulated bleb , hemorrhage. Conclusion AGV or combined AGV with other intraocular surgery is effective and relatively safe method for treating refractory glaucoma.
