摘要
目的研究大剂量抑肽酶对兔肺挫伤早期炎症反应的作用。方法24只新西兰大白兔随机分为3组(n=8),正常对照组、创伤组及治疗组,采用兔单侧肺挫伤生物撞击模型,观察伤前(0h)和伤后30min,1、2、4、6h血浆中TNF-α、IL-8、TXB2、6keto-PGF1α含量;6h后活杀动物后取支气管肺泡灌洗液(BALF)检测上述炎症介质及白蛋白含量和白细胞计数,肺组织湿/干重比(W/D)、丙二醛、髓过氧化物酶的变化;并对肺组织进行光镜病理观查。结果创伤组血浆及BALF中,TNF-αI、L-8、TXB2、6keto-PGF1α含量均明显升高(P<0.01或P<0.05),BALF中白细胞总数、PMN%、清蛋白明显增加(P<0.05),肺组织MDA和MPO的含量升高(P<0.01),肺组织损伤以肺出血、肺水肿和中性粒细胞浸润为主要表现。治疗组相应指标显著减少(P<0.05),肺组织损伤程度减轻(P<0.05)。结论早期应用大剂量抑肽酶能够抑制肺挫伤后的炎性反应。
Objective To investigate the effects of aprotinin on inflammatory reaction in the early phase of pulmonary contusion induced by impact chest trauma in rabbits. Mathods Rabbits were randomly divided into three groups(8 cases) : the normal control group , traumatic group and the aprotinin treatment group. The right lateral lung contusion model of the rabbits were established by bioimpact machine. The plasma concentration of TNF-α, IL-8, TXB2 and 6keto-PGF1α was detected at different time points of 0,0. 5,1,2,4,6h. The rabbits were killed at the end of 6h, The contents of TNF-α, IL-8, TXB2, 6keto-PGF1α and albumin and leukocyte counts were assessed in bronchalvelor lavage fluid (BALF). The wet/dry (W/D) weight ratio and the contents of Malondialdehyde (MDA) and myeloperoxidase (MPO) were analyzed in lung tissues. The histological features of lung were examined by light MICroscope. Results Compared with the normal control group, in traumatic group, the concentration of TNF-α , IL-8, TXB2 and 6keto-PGF1α were increased both in plasma and BALF(P〈0.01 or P〈0.05). The contents of albumin in BALF were higher(P〈0.01 or P〈0.05), The contents of MDA and MPO and the ratio of W/D were elevated in the lung tissue(P〈0.01 or P〈0.05). Pathological examination revealed a marked primary changes including alveolar hemorrhage, pulmonary edema, leukocyte extravasation and inflitration. Compared with the traumatic group, in the treatment group the markers above were significantly decreased (P〈0.05). Conclusion Aprotinin treatment could attenuate inflammatory reaction in the early phase of pulmonary contusion in rabbits.
