摘要
目的探讨高血压病患者动态脉压和主动脉根部内径(AOD)与左心室肥厚(LVH)之间的相互关系。方法对240例高血压病患者分别进行24 h动态血压监测和超声心动图检查。根据左心室心肌质量指数(LVMI),男>120 g/m2、女>110 g/m2作为左心室肥厚的标准,分为左心室肥厚组(94例)和无左心室肥厚组(146例)。结果24 h平均脉压、白昼平均脉压、夜间平均脉压和AOD在高血压左心室肥厚组和无左心室肥厚之间差异均有统计学意义(P<0.05)。经多元回归分析结果显示:夜间平均脉压和AOD与LVMI的关系最密切(P<0.001)。结论AOD和脉压、尤其是夜间平均脉压增大在LVH中起主要决定因素。
Objective To describe the relationship between ambulatory pulse pressure and left ventricular hypertrophy (LVH) and aortic mot dimension(AOD). Methods Two hundred and forty patients with essential hypertension were monitored 24 - hour ambulatory blood pressure and applied echocardiography. The patients were divided into two groups according to their left ventricular mass index (LVMI), LVH group (LVMI〉120 g/m^2 in man,and 〉110 g/m^2 in woman) were 94 subjects and non-LVH group 146 subjects. Results The average levels of 24 - hour pulse pressure, day time pulse pressure, night time pulse pressure and AOD were significantly different between patients with LVH and without LVH( P 〈0.05). Multiple stepwise regression analysis showed that the changes in night time pulse pressure and AOD were closely related to LVMI (P 〈 0. 001). Conclusion The pulse pressure and especially AOD are important to LVH in patients with essential hypertension.
