内镜下治疗胃肠道大息肉32例

Endoscopic Treatment of Large Gastrointestinal Polyps of 32 Cases

为探讨胃肠道巨大息肉内镜下电切术并发症的预防方法、适应症和效果，对32例共36枚胃肠道巨大息肉术前分别采用金属夹钳夹、尼龙圈结扎和黏膜下注射1：10000肾上腺索生理盐水。全部病例均治愈，未发生严重并发症。3种方法均能有效预防消化道大息肉电切术后出血和穿孔。金属夹钳夹适合蒂柄直径〈1．2cm的有蒂息肉，尼龙圈结扎适合蒂柄直径〉1．2cm的有蒂息肉。注射肾上腺素盐水适合广基息肉。

For the objective of probing into preventive methods for complication,indications and therapeutic effects of endoscopic electro-resection in treating large gastrointestinal polyps, the 32 cases（36 polyps） were subjected to （1） clipping by metal clips, （2） nylon snare ligation,and （3） submucosal injection of epinephrine-saline（1 ： 1 0000） before operation respectively. Results showed that all the patients were cured, without any severe complication. These three methods can all prevent bleeding and perforation effectively after the electro-resection;among which （1）method is suitable to the pedicled polyps of pedicle-haft diameter〈1. 2cm, （2）method to the polyps of pedicle-haft diameter〉1. 2cm, （3）method to the sessile type of polyps.
