72例喉癌患者耐药基因与临床预后关系的初步探讨

Primary Study and the Kelationship Between Drug Resistance Gene and Clinical Prognosis in 72 Pationts with Laryngeal Squamous Carcinoma

目的:探讨喉癌患者中耐药基因P糖蛋白(P-gp),谷胱苷肽-S转移酶(GST-π),DNA拓扑异构酶Ⅱ(TopoⅡ),胸苷酸合成酶(TS)表达的临床意义。方法:用免疫组化方法检测72例喉癌组织中的P-gp、GST-π、TopoⅡ和TS,分析其与临床病理资料及预后的相关性。结果:72例喉癌患者中,P-gp阳性66例、GST-π阳性50例、TopoⅡ阳性28例、TS阳性12例。P-gp的表达与病理分型、临床分期、淋巴转移及局部复发均相关,GST-π、TopoⅡ的表达与淋巴结转移相关,TS的表达与各项指标均无明显相关性。患者的病理分级、临床分期、术后是否复发及GST-π的表达均与术后生存时间明显相关。结论:喉癌患者P-gp、TopoⅡ、GST-π、TS的联合检测十分必要,病理分级、临床分期、术后复发及GST-π可以作为判断喉癌患者预后的指标。

Objective：To study the relationship between the expression of p-glycoprotein（p-gp）,glutathione-s-tranferase-π（GST-π）, topoisomerase Ⅱ （Topo Ⅱ ） and themidylate synthase（TS） and the clinical pathological characteristics of the patients with laryngeal squamous cell carcinoma. Methods.. The expressions of pgp,GST-π,TopoⅡ ,TS proteins were detected by immunohistochemistry method in 72 patiens with laryngeal squamous cell carcinoma who were followed up more than five years. Results：The expression of p-gp was associated with histological grading,UICC staging,lymph node metastasis and recurrence; The expression of GST- π and TopoⅡ was associated with lymph node metastasis; survival rate was correlated with histological grading, UICC staging, recurrence and GST-π. Conclusion.. Histological grading, UICC staging, recurrence and the expression of GST-πcan be redarded as progonosis ratters in laryngeal carcinoma.
