血小板成分细菌污染和预防 被引量:5

摘要 随着血小板成分在临床使用的日益增加,血小板细菌污染的问题越来越受到输血医学界的重视。血小板细菌污染的来源分为内源性和外源性两条途径,内源性污染可以通过健康征询,外源性污染则通过皮肤消毒、移去最初采集的部分血液等方法来防止。降低保存温度和减少保存时间也可减少细菌的生长繁殖。进行细菌污染检测可以降低血小板成分相关的败血症,检测的方法主要有漩涡试验、试纸条检测细菌新陈代谢参数、染色镜检、免疫学、分子生物学、流式细胞仪和全自动液体培养系统培养等。其中以全自动液体培养系统(BacT/ALERT和PallBDs系统)的灵敏度最高,可分别达到10CFU/ml和100~500CFU/ml。采用保存后去除白细胞、γ线照射、光化学灭菌等方法可以降低细菌载量。
