妊娠高血压综合征病人瘦素水平与糖代谢的关系

RELATIONSHIP BETWEEN LEPTIN LEVELS AND GLYCOMETABOLISM IN PREGNANCY-INDUCED HYPERTENSION

目的探讨血清瘦素水平与妊娠高血压综合征（妊高征）病人糖代谢的关系.方法对46例妊高征病人（妊高征组）及40例正常妊娠妇女（对照组）血清瘦素（LP）、空腹血糖（FBG）、空腹胰岛素（FINS）水平进行了检测,并对胰岛素抵抗指数（HOMA-IR）及胰岛素分泌指数（HOMA-IS）进行了分析比较.结果妊高征组血清瘦素水平明显高于对照组（t=8.100,P〈0.01）;中、重度妊高征组瘦素水平明显高于轻度妊高征组（F=8.312,q=3.82、4.37,P〈0.05）.妊高征组FINS水平、HOMA-IR及HOMA-IS与对照组比较明显升高（t=3.242～7.588,P〈0.01）.各组间FBG水平比较差异无显著意义（F=2.013,P〉0.05）.妊高征病人血清瘦素水平与FINS、HOMA-IR、HOMA-IS呈明显正相关（r=0.542～0.583,P〈0.01）.结论妊高征病人存在胰岛素抵抗及高胰岛素分泌,与血清瘦素水平升高密切相关.

Objective To investigate the relationship between the serum leptin levels and carbohydrate metabolism in patients with pregnancy induced hypertension （PIH）. Methods Serum leptin levels were measured by radio inmmnoassay in 46 patients with PIH （PIIt group） and 40 normal pregnant women （control group）. Fasting serum insulin （FINS） and fasting blood glucose （FBG） levels were determined, respectively. Insulin resistance index （HOMA-IR） and β-cell function index （HOMA-IS） were analyzed. Results Serum leptin levels in PIH women were significantly higher than those in normal pregnancy （t=8. 100, P〈 0.01 ）, Serum leptin levels in moderate PIH and severe PIH were higher than those in mild PIH women （F= 8.312 ;q= 3, 82,4.37; P〈0.05）. The levels of FINS, HOMA IR and HOMA-IS in PIH group were significantly higher than those in normal pregnancy group （t= 3. 242-7. 588, P〈0.01）, Serum glucose levels had no significant difference between different groups （F=2. 013, P〉 0, 05）. The levels of leptin in PIH were correlated to the levels of FINS, HOMA-IR and HOMA-IS （r =0. 542-0. 583, P〈0.01 ）. Conclusion Both insulin secretion and insulin resistance are elevated in PIH, and correlate to the increased serum levels of lepdn.
