缺血性脑血管病患者颅内外动脉狭窄的年龄分布特征 被引量:23

The distribution of intra and extracranial artery stenosis in different aged patients with ischemic cerebrovascular disease

摘要 目的:探讨缺血性脑血管病患者脑动脉狭窄的年龄分布特征,以指导脑血管病的治疗和预防。方法:观察行连续性主动脉弓和全脑血管造影检查的缺血性脑血管病患者142例,将其中存在脑动脉狭窄的108例患者按年龄分为青年组(15-44岁)、中年组(45-59岁)及老年组(≥60岁),并分析脑动脉狭窄随年龄变化的分布特点。结果:108例脑动脉狭窄患者中颅内、外动脉狭窄分别为69.4%和66.7%。青年组单纯颅内动脉狭窄的比例较高, 中年组及老年组颅内、外动脉狭窄并存的比例较高。青年组与中、老年组间狭窄类型比较的差异具有显著性意义 (P<0.05),中年组与老年组之间比较差异无显著性意义(P>0.05)。颅外动脉病变数目与年龄有关,而且随年龄增长而不断增加(P<0.01)。,颅内动脉病变数目与年龄无关。结论:缺血性脑血管病患者腩动脉狭窄的分布有年龄特征性,随着年龄的增长,颅外动脉狭窄数目逐渐增多。 Objective: To provide reference for the therapy and the prevention of cerebrovascular disease, we investigated the distribution of cerebral artery stenosis in different aged patients with ischemic cerebrovascular disease (ICVD). Methods : 108 of 142 patients examined with digital subtraction angiography had cerebral artery stenosis. The 108 patients were divided into young-aged group ( 15 - 44 ) years, middle-aged group(45 -59) years and old-aged group(≥60) years. The distribution of cerebral artery stenosis with different age was analyzed. Results: The frequency of intracranial artery stenosis was 69.4%, and that of extracranial artery was 66.7% in 108 patients with cerebral artery stenosis. In young-aged group, the proportion of simplex intracranial artery stenosis was "high, and middle cerebral artery was affected frequently. In middle-aged and old-aged group, the proportion of coexistence of intracranial and extracranial artery stenosis was higher. Compared with the frequency of stenosis, there was significant difference between three groups, and no significant difference between middle-aged and oldaged group. We found the correlation between the numbers of extracranial artery stenosis and age. The numbers of extracranial artery stenosis increased with age (P 〈0. 01 ). There was no correlation between the numbers of intracranial artery stenosis and age. Conclusion : The distribution of cerebral artery stenosis varied with different age in patients with ICVD. The numbers of extracranial artery stenosis increased with age.
