摘要 目的:探讨低场MRI在膝关节损伤病变的诊断价值。材料与方法:对关节镜或手术证实的84例104个膝关节损伤的MRI表现进行回顾性分析。结果:半月板损伤60个,其中1—2级42个,3级18个,3级损伤诊断符合率为79%(18/22)。前交叉韧带损伤23个,其中1-2级14个,3级完全撕裂9个,3级损伤诊断符合率为77%(7/9)。后交叉韧带损伤19个.其中1-2级12个,3级完全撕裂7个,3级损伤诊断符合率85%(6/7)。内侧副韧带损伤18个,其中1—2级14个,3级4个,3级损伤诊断符合率50%(2/4)。外侧副韧带损伤5个。关节软骨损伤23个,骨挫伤37个,骨折12个。关节腔内积液77个。结论:MRI能较好地显示膝关节的半月板损伤、韧带损伤、关节软骨损伤、骨挫伤及关节腔积液等改变,还能对半月板、韧带以及关节软骨的损伤进行分级,清楚显示膝关节损伤的病变特点和严重程度,为临床选择合适的治疗方案提供了依据。 Purpose: To study the value oflowfield MR imaging in the diagnesis of the trauma of the knee joints. Materials and Methods: The MRI findings of trauma of the 104 knee joints of 84 cases confirmed surgically or arthroscipically were retrospectively analyzed. Results: of 60 injuries of meniscus, degree 1 - 2 in 42, degree 3 in 18 the coincident rate of degree 3 is 79% (18/22) ; of 23 injuries of anterior cruciate ligament (ACL), degree 1 - 2 in 14, degree 3 in 9. the coincident rate of degree 3 is 77% (7/9) ; of 19 injuries of posterior cruciate ligament (PCL), degree 1 - 2 in 12, degree 3 in 7 the coincident rate of degree 3 is 85% (6/7) ; of 18 injuries of medial collateral ligament (MCL), degree 1 - 2 in 14, degree 3 in 4 the coincident rate of degree 3 is 50% (2/4) ; of 5 injuries of outboard coUateral ligament (OCL). Conclusion: low field MRI could dearly demonstrate the lesions in the knee joints, including the injuries of meniscus, ligaments articular cartilage, the contusion of bone and peripheral parenchyma and hydrarthresis. Furthermore low field MRI could grade the injuries of meniscus, ligaments, articular cartiolge. The diagnosis accuracy of MR imaging was superior to that of radiography. MR imaging could aeeruately demonstrate the features and severity of the trauma of the knee joints. MRI is an ideal and noninvasive technique in the diagnosis of the trauma of the knee joints and has important value in therapy planning.
