高血压左心结构改变与临床心功能分级的关系

Relation between Left Cardiac Structural Change of Hypertension and Clinical Classification of Cardiac Function

本文分析１５０例高血压病人结果提示：高血压引起左心结构的改变有左室肥厚（向心性，偏心性，离心性）及左房扩大。在左室向心性及偏心性肥厚的病人，心功能多为Ⅱ级，以左室舒张功能受损为主，收缩功能正常。而离心性肥厚的病人，心功能为Ⅲ－Ⅳ级，以收缩功能受损为主；单纯左房扩大是高血压早期心脏受累的指标，亦为舒张功能受损，心功能Ⅰ－Ⅱ级。年龄及病程对心功能有一定影响。

We analyses l50 patients with hypertension in this paper.The results suggest that changes of left cardiac structure caused by hypertension are hypertrophy of left ventricle(concentric,eccentric,centrifugal)and enlargement of left atrium. The cardiac function of patients with concentric and eccentric hypertrophy is gradeⅡ(NYHA classification),the diastolic function of left ventricle is injured,but the contractive function is normal.The cardiac function of patients with centrifugal hypertrophy is grade Ⅱ-Ⅳ,the contractive function is injured mainly;simple enlargement of left atrium is a sign of heart involvement in early stage of hypertension, the diastolic function is also injured,clinical cardiac function is gradeⅠ-Ⅱ.Age and course of patients with hypertension influence cardiac function including diastolic and systolic function.
