396例内窥镜鼻窦手术并发症的临床分析

396 Cases Clinic Analysis of Complications of Endoscopic Sinus Surgery

目的探讨如何稳妥开展内镜鼻窦手术，减少术中术后并发症发生，提高治愈率。方法回顾性分析1997-04-2004-05间396例（716侧）鼻内窥镜鼻窦手术中出现并发症的临床资料。结果本组396例（716侧）并发症发生率为16．8％。术中出现脑脊液鼻漏1例（1侧），眼眶内炎性肿块1例（1侧），眼内直肌损伤1例（1侧），术中需要输血的较大量出血32例（61侧），眶周瘀斑8例（9侧），术腔粘连33例（47侧）。结论加强鼻内窥镜手术操作训练，术前对病变性质、范围和手术方法作出充分的评估，以及术中精心操作是减少手术并发症和提高疗效的关键。

Objective To explore how to safdy carry out endoscopic sinus surgery and reduce the occurrence of complications at intraoperation and post - operation,lmprove clinical curative effect.Methods Analysis of the clinic data that had complications of endoscopic sinus surgery among 396 cases （716 sides） was made retrospectively. Results Total incidence complications was 16.8% .The types of complications inclnded cerebrospinal fluid （CSF） leak （ n = 1）, inflammatory pseudotumor in orbit （ n = 1）, eye muscle injury （ n = 1）, hemorrhage （ n = 61 ） periorbital emphysema （ n = 47）. Condusion Before operation, endoscopic sinus surgery training should be strengthened, pathologic featares and primary ailment severity must be assessed so that complications can be decreased and clinical curative effects can be achieved.
