颜面部皮肤癌的整复治疗

Plastic treatment of facial skin malignant tumor

目的探讨颜面部皮肤癌的手术治疗与面部形态及美容的关系。方法69例颜面部恶性肿瘤，根据病灶的部位和范围，扩大切除后行单纯缝合（14例）、游离植皮（11例）、局部皮瓣＋植皮（13例）、皮瓣转移（31例）等方法修复创面。结果皮瓣全部成活，皮片有3例表皮坏死，余皮片全部成活，切口愈合良好。原位缝合者无明艟术后瘢痕，随访6个月-3年，游离植皮者皮片颜色较周围皮肤深，稍影响外观，局部皮瓣＋植皮者位于面侧部、发际、耳后等隐蔽处，外观无明显影响，颜面部外观满意。结论颜面部恶性肿瘤宜早期手术扩大切除，选用合适的整复外科方法修复创面，可取得较满意的外形及预后。皮瓣应是修复创面的首选方法。

Objective To study the looks following plastic treatment of facial malignant tumor. Methods From 1998 to 2005, 69 patients with basal cell carcinoma and squamous cell carcinoma were treated in our department. Primary suture（ 14 cases）, local skin flap （31 cases）, skin graft （11 cases ） and skin flap plus skin graft （13 cases） were used to repair the wounds after extensive resection. Results The skin graft and various skin flaps all surived. The facial appearance and function were satisfactory after the surgery. Condusion Extensive resection of facial malignant tumor in early phase and suitable repair technique may help to preserve the appearance. Skin flap is the method of choice.
