胸腺瘤合并重症肌无力14例的外科治疗

Surgical treatment of 14 cases of myasthenia gravis with thymoma.

目的总结胸腺瘤合并重症肌无力的围手术期处理方法及外科治疗经验.方法回顾分析14例胸腺瘤合并重症肌无力患者手术治疗的临床资料.结果全组无手术死亡,术后早期发生肌无力危象3例,经气管插管呼吸机辅助呼吸等治疗,均抢救成功.术后随访0.5～5.0年,肌无力完全缓解4例,部分缓解7例,无改变3例.结论胸腺瘤合并重症肌无力宜采用手术、放疗、化疗等综合治疗,必须加强围手术期处理,积极防治并减少肌无力危象发生.

Objective To conclude experience of peri - operation and surgical, treatment for myasthenia gravis （MG） with thymoma. Methods The clinical data of surgical treatment for 14 cases of MG with thymoma had been analyzed retrospectively. Results None died in 14 cases . 3 eases complicated with myasthenla gravis crisis in early stage post - operation and were saved successfully after treatment of intubations and artificial respiratory machine assisted respiratory. All the cases were followed up 6 months to 5 years, 4 cases of MG relieved completely, 7 cases relived partially and 3 cases didn＇t change. Conclusion The comprehensive treatment including operation ,radiotherapy and chemotherapy should be considered for MG with thymoma, strengthening the management of peri - operation , preventing and reducing the myasthenia gravis crisis are good to the recovery of oatient.
