内窥镜下泪囊鼻腔造孔术治疗慢性泪囊炎

Intranasal Endoscopic Dacryocystorhinostomy for Chronic Dacryocstitis

目的:探讨鼻内窥镜下泪囊鼻腔造孔术治疗慢性泪囊炎的疗效。方法:18例(22眼)慢性泪囊炎患者,在全身麻醉下,00硬管鼻内窥镜指引下,实施泪囊鼻腔造孔术,术中不置管。同时7例合并鼻腔、鼻窦病变(鼻息肉、慢性鼻窦炎2例,鼻中隔偏曲5例),同时进行相应手术治疗。结果:术后随访3至18个月,治愈12例(15眼),好转4例(5眼),无效2例(2眼),有效率为90.9%。无明显并发症。结论:内窥镜下泪囊造孔术是治疗慢性泪囊炎的理想方法,可避免颜面部切口、损伤小、疗效高。同时可处理鼻腔、鼻窦疾病。

Objective： To analyze the outcome after intranasal endoscopic dacryocystorhinostomy. Methods： Endoscopic dacryocystorhinostomy was performed in 22 sides of 18 patients who had chronic epiphora. No silicone tubing was used in any of the cases. Among them 7 with deviation of nasal septum or nasal polyps and chronic sinusitis were treated at same time. Results： After a follow-up of 3 to 18 months, 15 out of 22 sides of the patients were free of symptoms. Four additional patients（5 sides） became symptom-free after surgery. The total success rate was 90.9%. Conclusion： Intranasal endoscopic dacryocystorhinostomy is a rapid, safe, and elegant method for the treatment of chronic dacryocstitis.
