P63、34βE12及PCNA表达在乳腺良恶性上皮性病变鉴别诊断中的意义

The diagnostic value of P63,34βE12 and PCNA in differential diagnosis of epithelial benign and malignant breast diseases

目的:探讨肌上皮标记物P 63、高分子细胞角蛋白34βE 12、增殖细胞核抗原(PCNA)表达在乳腺良恶性上皮性病变鉴别诊断中的意义。方法:用免疫组组织化学法(LSAB)检测肌上皮标记物P 63、高分子细胞角蛋白34βE 12及增殖细胞核抗原(PCNA)在45例乳腺肿瘤性增生病变、10例导管内癌及原位乳头状癌,26例浸润性癌中的表达,分析其与乳腺良恶性上皮性病变的关系及意义。结果:在45例肿瘤性增生病变、10例导管内癌及原位乳头状癌中,肌上皮表达均为阳性,26例浸润性癌,肌上皮均为阴性。34βE 12的表达率及其强度在良性病变与癌组之间,差异有显著性(P<0.05)。在良性病变与癌组之间,PCNA阳性表达率及表达程度差异有显著性(P<0.05)。结论:P 63、34βE 12可作为乳腺病变鉴别诊断中良性病变的标记物,而PCNA阳性表达,反应细胞增殖活性高,有助于良恶性病变的鉴别。

Objective：To study the diagnostic value of P63,34βE12,PCNA in differential diagnosis of epithelial breast benign and malignant diseases. Methods：Using immunohistochemistry（LSAB）method,the expression of P63, 34βE12,PCNA was determine in 45 cases of breast benign proliferative diseases, 10 cases of breast intraductal carcinomas and 26 cases of breast invasive carcinomas. Results：The 45 cases of breast benign proliferative diseases and 10 cases of breast intraductal carcinomas were either positive for P63,the 26 cases of breast invasive carcinomas showed either lack of P63. A significant difference was found between the expression of 34βE12 in breast benign disease and breast carcinoma （P〈0. 05）. There were significant difference with the frequency and strongness of PCNA in breast benign disease and breast carcinoma. Conclusion：P63 and 34βE12 may serve as a marker of breast benign disease for differential diagnosis, PCNA can help with the differential diagnosis of epithelial breast benign and malignant diseases.
