老年人腹腔镜胆囊切除72例临床体会

The Clinical Experience of the Elderly Cholecystectomy with Laparoscopic in 72 cases

目的探讨老年人行腹腔镜胆囊切除术(LC)的治疗效果。方法回顾性分析72例行腹腔镜胆囊切除术的老年患者的临床资料及治疗效果。结果应用LC治疗72例老年胆囊结石、胆囊息肉患者,因胆囊与周围组织重度粘连而中转开腹2例,余70例均成功行CL治疗,LC成功率为97.2%。结论LC创伤小、对腹腔干扰少、术后恢复快,适合治疗老年性胆囊疾病;但要注意围手术期处理,果断中转开腹是预防并发症的重要措施。

Objective To discuss the curative effect of elderly cholecystectomy （LC） with laparoscopic,Methods The clinical data and curative effect of 72 old patients＇ LC were reviewed, Results LC was used to cure 72 patients of elderly cholelithiasis, cholecystolithiasis and cholelithiasis polyp, The bellies of 2 cases were cut open during surgery because the tholecyst and surmunding tissue accreted heavily, the rest 70 cases were cured successfully through cholecystectomy. The successful rate of cholecystectomy was 97.2%. Conclusion Cholecystectomy posseses some advantages such as small wound, little pain, few interference with abdominal cavity, quick postoperative recovery, which fits for elderly cholecyst disease. Dealing with the perioperative management and changing into cutting the belly open are the key measures to prevent complication.
