鼻内特异性免疫疗法治疗常年性变应性鼻炎的临床及免疫学观察

The Clinical and Immunologic Study on Nasal lmmunotherapy for Perennial Allergic Rhinitis

目的探讨鼻内特异性免疫疗法治疗常年性变应性鼻炎的效果及机制。方法用鼻内特异性免疫疗法治疗常年性变应性鼻炎413例,并与皮下特异性免疫疗法进行疗效比较,同时检测其中50例的相关免疫学指标。结果鼻内特异性免疫疗法总有效率达85.9%,与皮下特异性免疫疗法疗效无显著差异(P>0.05);治疗后鼻分泌物中的 SlgA 水平增高,与治疗前比较.差异有统计学意义(P<0.05);治疗后鼻分泌物总lgE、血清总 lgE 及螨、真菌等特异性 lgE 水平呈下降趋势,但与治疗前比较,差异无统计学意义(P>0.05)。结论鼻内特异性免疫疗法疗效好、安全方便、无副作用,可替代皮下特异性免疫疗法;其机制可能是增强了鼻腔局部免疫功能。(中国眼耳鼻喉科杂志,2006,6:157～159)

Purpose To investigate the clinical efficacy and immune mechanism of nasal special immunotherapy （NSIT）. Methods Four hundred thirteen cases of perennial allergic rhinitis （PAR） were treated with NSIT. ImmunoLogic study was carried out in 50 cases. Results Total effective rate of NSIT were 85.9%, there were no significant difference between NSIT and subcutaneous immunotherapy （ P 〉 0.05）. Immunologic study demonstrated, that the nasal secretory IgA （S-IgA） increased significantly after treatment （ P 〈 0.05）. The levels of nasal secretory total IgE, serum total IgE and serum specific IgE（anti-mite, mildew） decreased, but it was no significance（ P 〉 0.05）. Conclusion NSIT is an effective, safe and convenient treatment. It can improve the local nasal immune function.
