摘要
目的应用彩色室壁运动定量分析技术评价冠状动脉旁路移植术前后左室节段心功能的变化。方法26例行冠状动脉旁路移植术(CABG)的冠心病患者和20例正常对照者应用彩色室壁运动技术和图像处理软件(ICK)检测左室各节段收缩与舒张功能,观察冠心病患者CABG术前后CK和ICK定量分析条形图、曲线图以及定量指标收缩指数(SI30、SI60、SI100)与舒张指数(DI)的变化,并与正常组比较。结果正常对照组左室各节段收缩与舒张运动呈现同步性,各节段SI和DI差异无统计学意义(P>0.05)。26例冠心病患者术前节段运动明显不同步,狭窄相关节段SI与DI较正常对照和非狭窄相关节段明显降低(P<0.05)。术后,手术相关节段的SI和DI随时间推移逐渐增高,与术前比较差异有统计学意义(P<0.05)。结论彩色室壁运动定量分析技术能定量评价节段心室功能,为冠心病的诊断和疗效评价提供准确、可靠的依据。
Objective To evaluate the application of color kinesis (CK) in quantitative analysis of regional left ventricular function changes before and after coronary artery bypass grafting (CABG) surgery. Methods Twenty-six coronary artery disease (CAD) patients and twenty normal controls were examined using CK and ICK software to analyze regional left ventricular systolic and diastolic function. Systolic indexes(SI30, SI60, SI100 ) and diastolic index (DI) of patients and controls were obtained and compared between groups and different time points. Results Systolic and diastolic synchrony of left ventricular regions were shown in twenty normal controls, there were no significant differences between regions ( P 〉0. 05). Wall motion asynchronies were seen in patients before surgery, and the SI and DI were significantly lower in stenosis-related regions than in normal controls and non-stenosis regions ( P 〈0.05). After CABG, the SI and DI of surgery related regions increased gradually and were different between post- and pre-operation ( P 〈0. 05). Conclusions CK and ICK quantitative analysis system can evaluate the regional left ventricular function quantitatively, provide new and accurate parameters in effective evaluation and diagnosis of CAD.
