注射用血凝酶对外科手术止血的量-效研究

Quantity-Effect Research of Hemostatic Effect of Reptilase for Injection in Surgical Operations

目的:研究术中、术后使用不同剂量注射用血凝酶对外科手术止血的量-效关系。方法:将120例泌尿外科及普通外科手术患者随机分为实验组与对照组,试验组术中静脉注射注射用血凝酶2KU,术后静脉滴注或静脉注射注射用血凝酶1KU～2KU,2次/d,至创面出血停用;对照组术中静脉注射注射用血凝酶1KU,术后静脉滴注或静脉注射注射用血凝酶1KU,1次/d,至创面出血停用。结果:试验组与对照组的出血量分别为(141±11.2)ml、(198±12.1)ml(P<0.05),术后血红蛋白分别为(12.1±0.4)g/L、(11.1±0.3)g/L(P<0.05)。结论:与常规剂量比较,大剂量使用注射用血凝酶更能有效地预防术中、术后出血。

OBJECTIVE ： To study the quantity - effect relationship of reptilase with surgical hemostasis used at different dosages perioperatively and postoperatively.METHODS： 120 patients undergoing urological and general operations were randomly divided into experimental and control group. In the experimental group reptilase was administered at the dose of 2KU by iv during the operation and continuously given at the dose of 1KU-2KU by iv drip or iv twice a day after the operation until bleeding stopped. In the control group reptilase was administered at the dose of 1KU by iv during the operation and continuously given at the dose of 1KU by iv drip or iv once a day after the operation until bleeding stopped .RESULTS： The bleeding volumes in experimental and control group were（ 141 ± 11.2） ml and （ 198 ± 12.1） ml（P〈 0.05）, respectively, and the postoperative hemoglobins were（ 12.1 ± 0.4）g/L and （ 11.1 ± 0.3）g/L（P 〈 0.05）, respectively .CONCLUSION： Large dose of reptilase by iv can have better effects in preventing perioperative and postoperative bleeding,as compared with conventional dosage.
