术前动脉灌注化疗对Ⅲ期非小细胞肺癌切除率的影响(附60例分析)

The Effect of Preoperative Bronchial Artery Infusion Chemotherapy in Lung Cancer

目的分析术前支气管动脉灌注化疗对Ⅲ期非小细胞肺癌（NSCLC）的疗效。方法对32例Ⅲ期NSCLC患者（Ⅲa期21例、Ⅲb期11例），术前采用MVP及NP方案行支气管动脉灌注化疗，灌注化疗后3—4周内行手术治疗。结果32例中CR9．3％（3／32）；PR37．5％（12／32）：NC53．2％（17／32）。26例施行了手术切除，总切除率81．3％。结论术前动脉灌注化疗可降低Ⅲ期NSCLC病理分期，提高山期NSCLC的手术切除率。

Objective We carried preoperative bronchial artery infusion chemotherapy in 32 patients with stage Ⅲ non-small cell lung cancer （NSCLC）. Bronchial artery infusion chemotherapy could improve the OR rate in advanced lung cancer. Therefore, the preoperative bronchial artery infusion chemotherapy for stage Ⅲ NSCLC should be investigated. Method Twenty one patients with local advanced NSCLC （21 with stage Ⅲ a stage and 11 with stage Ⅲ b stage ） were enrolled into the study. The regimen consisted of mitomycin （10mg/m^2）, VDS（3 mg/m^2）, DDP（60 mg/ m^2）, or NVB（25 mg/m^2）, DDP（60 mg/m^2）, which were given through bronchial artery on the first day. Then the surgical resection was performed approximately on the 28th day. Results 3 patients （9.3 % ） had complete response and 12 patients （37.5 % ） had partial response. 17 patients had no change to the chemotherapy. After the chemotherapy, the surgical resection was performed on 26 patients （81.3 % ）. Conclusion Preoperative bronchial artery infusion chemotherapy can contribute to high response rate and improve resectability in stage Ⅲ NSCLC.
