DF生化调节方案治疗晚期食管癌疗效观察

摘要 目的观察小剂量顺铂联合5-Fu持续滴注组成的生化调节方案治疗晚期食管癌的疗效及毒副反应,评价临床应用价值。方法70例晚期食管癌按入院先后随机分为治疗组38例及对照组32例,治疗组DDP 10 mg/m25、-Fu 0.5 g加液体持续滴注12 h,连用10天;对照组DDP 20 mg/m25、-Fu 1.0g加液体持续滴注12 h以上,连用5天,21天为一周期,连用3周期。结果治疗组CR 2.63%,有效率57.89%,对照组CR 3.13%,有效率53.12%,两组对比无统计学意义(P>0.05);对照组恶心呕吐反应及白细胞下降程度均高于治疗组(P<0.05),两组均未发现其它严重毒副反应。结论DDP联合5-Fu治疗晚期食管癌剂量10天分割治疗与5天分割治疗客观疗效无差异,但前者毒副反应小,患者生活质量改善明显,不须住院,更加经济。 Objective To observe the effects and bad effects of biochemical modification composed of low dose Cisplatin (DDP) combined with continuous infusion of 5-fluorouracil (5-Fu) for advanced esophageal carcinoma, and to evaluate the clinical practical value. Methods 70 cases with advanced esophageal cancer were divided into two groups randomly according to the sequence of hospital admission they were testing group (38 cases) and control group (32 cases). The testing group with 0.9% NS of DDP 10mg, 5-Fu O.Sg, continuous infusion 12 hours lasting for 10 days; the control group with 0.9% NS of DDP 20mg, 5-Fu 1.0g continuous infusion over 12 hours lasting for 5 days, 21 days as a cycle, continuous therapy 3 cycles. Results In the testing group, complete response (CR) rate was 2.63%, effect rate was 57.89%. In the control group, complete response (CR) rate was 3.13 %, effect rate was 53.21%, and two groups had the same effects ( P 〉 0.05) . Nausea, vomiting, leukopenia, etc. in the control group had happened more than those in the testing group ( P 〈 0.05) . No serious toxicity was found in two groups. Conclusion there was no significant difference in objective efficiency between 5 days and 10 days treatment with DDP and 5-Fu for advanced esophageal carcinoma, but the less serious toxicity was observed in the first group, the quality of life was improved significantly, no need for hospitalization and less expensive.
