β3肾上腺素能受体基因Trp^(64) Arg突变与胰岛素抵抗相关 被引量:1

Study of the association between β3 adrenergic receptor gene Trp^(64) Arg mutation and type 2 diabetes

摘要 目的采用聚合酶链式反应-限制性酶切片段长度多态性(PCR-RFLP)技术,探讨β3肾上腺素能受体(β3 adrenergic re-ceptor,β3AR)基因Trp64Arg突变与中国人胰岛素抵抗(IR)的相关性。方法用PCR-RFLP技术,检测2型糖尿病组(DM组)和正常对照组(N组)β3AR基因型,并测量血糖(FPG)、血浆胰岛素(FINS)等临床生化指标。以FPG×FINS/22.5计算胰岛素抵抗指数(HO-MA-IR),以BM I<25的健康人HOMA-IR的75分位值为界,将DM组分为胰岛素相对敏感组及相对抵抗组。结果与无突变者相比,β3AR Trp64Arg突变携带者BM I升高,2组间WHR、血脂、血压、心率、病程及发病年龄差异无统计学意义。胰岛素相对抵抗组68例,占DM组的56.7%,提示2型DM患者大部分存在IR。胰岛素相对抵抗组β3AR Trp/Arg基因型频率明显高于胰岛素相对敏感组,提示β3AR Trp64Arg基因突变参与IR的发生。结论β3AR Trp64Arg突变参与中国人体重的调节,可能是IR的危险因素之一。 Objective To investigate the relationship between β3 adrenergic receptor (β3AR) gene Trp^64Arg mutation and insulin resistance (IR) in Chinese. Methods The 120 patients with type 2 diabetes mellitus (group DM ) and 80 nondiabetic subjects as normal controls( group N)were involved in the study. Polymerase chain reaction(PCR) and the restriction fragment length polymorphism analysis (PCR-RFLP) were used to determine the β3AR genotype. The indexes such as body weight, height, waist circumstance, hip circumstance, blood pressure, lipids, fast plasma glucose and insulin were detected in all subjects. The IR was estimated by homeostasis model assessment (HOMA-IR). The patients in the diabetes group were divided into relative insulin sensitive group and relative insulin resistant group by the 75th percentile HOMA-IR. Results The BMI of the patients with the β3AR Trp^64Arg mutation was significantly higher than that of those without the mutation ( P 〈 0.05 ). Between the patients with and without the β3 AR Trp^64 Arg mutation, there was no significant difference in WHR, serum lipids, blood pressure, heart rate and course of disease. There were 68 relative insulin resistant subjects ( 56.7% ) in DM group. The β3AR Trg/Arg genotype frequency in relative insulin resistant group was significantly higher than that of relative insulin sensitive group (45.6% vs. 26.9%, P 〈0. 05), which indicated that the β3AR Trp^64Arg mutation was involved in the occurrence of IR. Condusion In Chinese, the Trp^64Arg mutation in β3AR gene contributes to the variation of weight and is a risk factor to IR.
