重症口腔颌面部多间隙感染的综合处理

Comprehensive management of severe oral and maxillofacial multiple space infections

目的:探索内、外学科合作诊治重症口腔颌面部多间隙感染的方法,以提高临床治愈率。方法:重症口腔颌面部多间隙感染10例,由急诊科与口腔颌面外科协作诊治,给予切开引流、抗感染、全身营养支持治疗,对其临床表现、实验室检查和治疗结果等进行回顾分析。结果:经内、外科协作治疗后,7例痊愈,3例(30.0%)死亡,较传统治疗死亡率降低(61.5%)。结论:对重症口腔颌面部多间隙感染,采取内、外科协作,全身与局部兼顾的方法综合处理,可降低死亡率,提高治疗效果。

PURPOSE： To evaluate the results of muhidisciplinary management of severe oral and maxillofacial multiple space infections with the aim of improvement of the clinical efficacy. METHODS： 10 cases of severe oral and maxillofacial multiple space infections were treated by muhidisciplinary approach involving emergency department, and oral and maxillofacial surgery department of our hospital. The approaches included establishment of patent drainage, anti- infection, anti-shock as well as nutrition support. The clinical data, laboratory data, diagnosis and treatment procedures were retrospectively analyzed. RESULTS： Among 10 cases with severe oral and maxillofacial multiple space infections, 7 were cured after cooperative treatment while 3 died. The rate of mortality （30.0%） was lower than that of traditional treatment（61.5%）. CONCLUSION： For severe oral and maxillofacial multiple space infections,better clinical results and lower mortality can be achieved by cooperative management including systematic and local treatment.
