直肠癌根治术患者手术体位的改进

An Alteration of the Operating Position for the Radical Resection on Patients with Rectal Cancer

目的探讨改进直肠癌根治术的体位摆放方法,最大限度满足患者的舒适和安全。方法将58例行直肠癌根治术的患者随机分为两组,A组采用传统方法,B组采用改进方法(改进搁脚架的高度与角度及托腿板的角度与方向)。观察两组体位摆放时间、术后舒适度、手术野暴露情况。结果B组患者体位摆放时间短于A组,差异非常显著(P<0.01);术后舒适度B组优于A组,差异显著(P<0.05);术者对两组患者的手术野暴露情况均满意。结论改进后的手术体位护理人员易掌握,且省时、省力,患者术后舒适度改善。

Objective To explore the alternative operating position that could maximize the comfort and safety for patients on operating table for cancer excision in the lower rectum. Methods Fifty-eight patients undergoing radical excision with rectal cancer were randomly divided into 2 groups. Group A was traditional position referred to conventional lithotomy position as controls. Group B was alteration position, with altered height, angle and the direction of the thighs-flat and leg pad. The time of setting patient＇s position, post - surgical comfortable degree, and visualization of operation area were compared between the 2 groups. Results The setting time in group B was significantly shorter than that in group A（ P 〈0.01 ）. The patients in group B felt more comfortable than those in group A after operation （ P 〈 0.05 ）. Both the two groups had satisfying visualization of operation area. Conclusion The altered operating position is more useful and easier to operate. It could save time and force for nurses at operating room. Moreover, the patients felt more comfortable after being operated.
