摘要
目的探讨心脏不停跳左房室瓣置换术中应用氨溴索(沐舒坦)肺保护的效果。方法风湿性心脏病患者36例随机分为实验组(应用沐舒坦)与对照组(不用沐舒坦)。体外循环(CPB)90min。于6个时点采静脉血,分别检测检测血浆血栓素(TXB2)和前列腺环素(6-keto-PGF1a)。结果两组CPB期间TXB2与CPB前比较均有不同程度升高,实验组与对照组TXB2和6-keto-PGF1a在CPB中各时点差异均有统计学意义(P<0.05)。结论TXB2可作为CPB过程中检测肺保护效果的敏感指标;心脏不停跳左房室瓣置换术中应用氨溴索(沐舒坦)可以减少TXB2的释放,具有较佳的肺保护效果。
Objective To study the predictive effect of ambroxol in replacement of mitral valve in beating heart. Methods Thirty-six patients underwent mitral valve replacement were randomly divided into experimentel group( use ambroxol in beating heart, n = 18) and control group(use accrue-brinein beatingheart, n = 18 ). Cardiopulmonary bypass(CPB) was performed for 90 minutes. Plasma thromboxane B2 (TXB2) and epoprostenol (6-keto- PGF1a) of tow groups were respeptively measured and contrasted at six different time intervals. Results TXB2 in control group increased more quickly,6-keto-PGF1a in experimental group increased more quickly. Conclusion Plasma TXB2 may be a sensitive detective guideline in lung protective effects. Beating heart is propitious to reduce the release of TXB2 ,and to increase the release of 6-keto-PGF1a.
