带蒂胆囊瓣或空肠壁修补肝外胆管缺损16例报告

Experience of gallbladder pedlcule flap graft or intestinum jejunum film to repair the extrahepatic duct dect defect

目的 探讨肝外胆管缺损简捷、有效的修补术式。方法用带蒂胆囊瓣修补或带蒂空肠壁转移修补肝外胆管缺损16例。其中Mirizzi综合征Ⅱ、Ⅲ型致肝外胆管缺损14例；原发性胆总管结石、胆总管坏死穿孔2例。结果术后带T形管3～6个月，经T形管造影证实无结石后拔管，全组病例术后均未出现并发症，预后良好。结论用带蒂胆囊瓣修补或带蒂空肠壁转移修补肝外胆管缺损，手术简单易行，取材方便，合乎胆管生理，临床效果满意。

Objective To explore the simple and effective operative treatment to repair the extrahepatic duet defect. Methods Gallbladder pedicle flap graft or jejunum pediele flap graft was used to repair 16 cases with extra hepatic duet including hepatic common duet injury of type Ⅱ , Ⅲ Mirizzi syndrome for 14 cases and the primary commonbile duct stones with ulceration and perforation of commonbile duct wall for 2 eases. Results The T-tube could be extratcd 3-6 months postoperation when the rcsults of eholangiography were nomal. Conclusion The treatment of the gallbladder pedicle flap graft or jejunum pedicle flap graft to repair the extrahepatic duct defect is simple and conforms with the physiology of bile duct and its materials are convenient to gain.
